Leyden District 212 will be hosting Residency Days at each campus for incoming freshmen in the District and current students, for the upcoming 2017-2018 school year. State law requires that school districts must ensure the students attending the school district live within the boundaries. Verifying residency for students ensures that our schools are compliant with state law.
Residency Days have been scheduled for:
Thursday, January 26, 2017, from 7:30 a.m. to 6:30 p.m.
Saturday, January 28, 2017, from 8:00 a.m. to 12:00 p.m.
It is imperative that you attend one of the Residency Days. Failure to provide ALL documentation may result in your student being excluded from the registration process.
At East Leyden, enter the building using Entrance 4 and at West Leyden, enter the building using Entrance 2. Please complete and bring the residency form(s) and ALL necessary proof of residency documents. Notaries will be available to sign the needed forms on these dates. Incomplete documentation will prevent you from participating in the Residency Days.
If you are renting without a lease, the Landlord form must be filled out by the owner and notarized.
If you are sharing a residence, you and the owner must fill out an Affidavit form.
FRESHMAN ONLY - If you are divorced or separated, and do not have legal custody documents, please complete Form 3 - Affidavit for Divorced or Separated Families.

We will be taking photos for incoming freshmen ID's from 8:00 a.m. to 12:00 p.m. on Saturday, and 4:00 p.m. to 6:30 p.m. on Thursday.
